Kathleen Conderato, DDS
Dr. Conderato has been practicing dentistry since graduating from Case Western Reserve Dental School in Cleveland in 1988. She served 3 years of active duty with the Navy as a general dentist in Great Lakes, Il achieving the rank of Lt. Commander. She opened a private practice in Mansfield, OH living there for 20 years before selling the practice and fulfilling a life-long dream of relocating out west. Dr. Conderato now lives in Cave Creek and enjoys a life with her spouse and 3 very active cattle dog mixes. She enjoys working out, running, mountain biking and hiking. She has recently discovered the joys of traveling with an RV and loading up the dogs every chance she gets for fun and adventure. She feels blessed to be a part of the dental family at Grand Avenue Family Dental and looks forward to many more years of dentistry here in Surprise.
Saif Ghanem, DDS
Dr. Ghanem’s love of the outdoors and his active lifestyle is one of the many reasons he moved to Arizona. In his free time, you’ll find him hiking, playing sports, traveling, and checking out new local restaurants.
Dr. Ghanem finds pleasure in being a General Dentist and how it joins both the world of art and healthcare.
He is skilled in a variety of dental treatments to include Crown and Bridge, Root Canals, Surgical Extractions, and restorative smile makeovers. Dentistry gives him the chance to work in a team environment and he loves the instant gratification of making his patients smile. He would love to have the chance to help you achieve your dental goals.
Dr. Ghanem attended the University of Michigan where he earned his Bachelor of Science in Biomolecular Science. He then went on to the University of Michigan School of Dentistry where he received his degree in Doctor of Dental Surgery (DDS)
Bree-Practice Manager
Bree joined the Grand Avenue Family Dental crew in 2013 and brings a wealth of experience with over 14 years as an Office Manager. She has always loved helping people smile! She is originally from Washington state, but has lived here in Arizona with her husband going on 11 years. Their 5 kids keep them busy with school & sports. They have 2 cats, Jazzie & Cena, who complete their family perfectly! In her spare time, you’ll find her interior decorating, active with Boxing & Kickboxing or watching true crime shows.
Jennifer- Dental Hygienist
Jennifer moved from the Chicago suburbs in 1997. She has been a dental hygienist since 2014 and has loved the dental field since she was a teenager in braces. She enjoys getting to know all her patients and putting any fears or anxiety they might have to rest. She is laser certified with numerous hours of CE courses under her belt. Her main focus is patient comfort and excellent gingival and periodontal care. Every other weekend you can find her at the racetrack watching her son compete in Karting. She enjoys traveling, working out at the gym, cooking, dining, and most of all spending time with her family.
Stacy-Administrative Assistant
Stacy began her career as a dental receptionist in 2017. She relocated to Arizona in the spring of 2020 from Hilo Hi. Stacy has worked as a dental assistant from 2000 to 2004, and from 2014 to 2017 She is committed to assist the office manager and team to provide the best care for our patients. Stacy has been married for 17 yrs and has two boys, the oldest is Nate who is 17yrs old and the youngest is Zach who is 12yrs old. She loves the outdoors, spending time with her family, and traveling.
Sarah-Dental Assistant
Sarah has been a dental assistant for over 8 years and graduated from Carrington College in Phoenix. She enjoys being part of a great team that truly cares about our patients. She loves having the opportunity to help our patients achieve a healthy and happy smile. When she is not at work her time is spent with her three children. She loves making memories and anything to do with the outdoors; swimming, hiking and traveling. Sarah is always smiling and is a very positive person.

Rachel-Dental Hygienist
Rachel a graduate of West Los Angeles College Dental Hygiene Program, in Los Angeles, California. I have 20 years experience working as a Registered Dental Assistant & Registered Dental Hygienist . I enjoy my career where I strive for excellent patient communication, patient rapport and quality of care. I graduated president of my class in 2016 and was honored in the top five. I am actively involved with dental hygiene outside of the clinical practice, by being a president of my local and state dental hygiene organizations, as well as a national product educator. Outside of the office I love to spend time with my five horses, where I actively compete in barrel racing. I also enjoy spending time on the lake, hiking and rollerblading. I look forward to seeing you soon.
